About Blossom Pathways

Every person arrives with a story already in progress.

We believe that story still has room for connection, purpose, and beautiful everyday moments.

The person comes first

A life is more than a diagnosis.

At Blossom Pathways, we recognize that we are all unique, with our own life stories. Those stories and interests shape who we are — and how we interact with others and our environment throughout the dementia process.

Our work begins with listening. To the caregiver. To the person living with dementia. To the routine, relationships, and unique details that make life feel like their own.

This is not the end of your story.

Let’s keep writing the book — and make it a good one.

About me

Chelsea Zimerle, Occupational Therapist.

Blossom Pathways is owned and cared for by Chelsea Zimerle, an Occupational Therapist with over a decade of experience supporting hundreds of individuals living with dementia. Throughout her career, Chelsea recognized a critical gap in healthcare, specifically support for the caregivers who are devoted to caring for their family member living with dementia.

Built on research-backed and certified techniques, Blossom Pathways took root to bridge that gap. At our core, we believe life can remain deeply meaningful, beautiful, and fruitful at every stage of the dementia process.

What guides us

Curiosity. Connection. Collaboration.

Stay curious

Behavior is communication. Before we try to manage anything, we ask what it is telling us — and what need is waiting to be met.

Protect connection

Relationship outlasts memory. The way we approach, speak, and share a moment can help keep that relationship alive.

Build it together

The caregiver and the loved one living with dementia both help shape the plan. Both voices matter, at every stage.

A gentle first step

You don’t have to do this alone.

Let’s talk about your person, your everyday challenges, and what support could make possible.
